Egy 35 éves férfi torokfájás, háti fájdalom, köhögés, köpetürítés, lázas állapot miatt kereste fel a sürgősségi osztályt. Állapota és magas gyulladásos paraméterei hátterében nem tudtak infekcióforrást azonosítani. Fül-orr-gégészeti konzílium során a klinikai kép mediastinitisszel szövődött, mély nyaki fertőzés lehetőségét vetette fel. Az elvégzett komputertomográfiás vizsgálat ezt megerősítette, ezért azonnali intenzív osztályos elhelyezése mellett sürgősséggel a műtőbe mentünk. A nyaki fasciakompartmentek feltárása mellett a jugulum felől és a retropharyngealis térségen keresztül a mediastinum drenázsát biztosítottuk az adekvát mellkassebészeti beavatkozás időpontjáig (kolláris mediastinotomia). A további ellátás is aktív közreműködésünkkel zajlott. Esetbemutatásunk célja, hogy rávilágítson a fül-orr-gégész, fej-nyak sebész által betöltött szerepre ezen életveszélyes állapot multidiszciplináris ellátásában.

The head and neck surgeon’s role in the management of deep neck infections complicated by mediastinitis. Apropos of a case.
Summary
A 35 years old man visited our emergency department because of sore throat, back pain, cough, sputum and fever. Unfortunately, they were unable to identify the underlying septic focus of his general condition and elevated inflammatory parameters. During the otorhinolaryngology examination, we raised the possibility of a deep neck infection complicated by mediastinitis, and it was confirmed by computed tomography. Thus, we immediately admitted the patient to the intensive care unit and took him to the operating theatre. Besides the cervical fascia compartments’ exploration, we drained the mediastinum via transcervical approaches from the jugulum and through the retropharyngeal space until the thoracic surgeons’ adequate intervention. We were also involved in further treatment of the patient. The goal of our case report is to highlight the otorhinolaryngologists and head and neck surgeons’ role in the multidisciplinary management of such a life-threatening condition.
