Összefoglaló
Bevezetés: A különböző etiológiájú, veleszületett vagy szerzett (krónikus irritáció, iatrogén ártalom) hallójárati atrézia műtéti megoldására számos sebészi eljárás létezik. Jelen esettanulmányukban egy kiterjedt, a cimpára, a conchára, valamint csaknem a teljes hallójáratot érintő, marószer okozta sérülés és következményes atrézia műtéti rekonstrukciós megoldását mutatjuk be.
Célkitűzés: Olyan egyedi, sebészi módszer megtervezése és kivitelezése, amely egyszerre alkalmas a hallójárat-bemenet, a tragus és a cimpa nagy kiterjedésű hámhiánnyal járó sérülésének esztétikai rekonstrukciójára és a heges középső és külső hallójárati atrézia oldására, valamint az érintett területek funkcionális hámborításának kialakítására.
Módszer: A heges szűkület eltávolítását követően a hallójárat csontos falának fúróval történő tágítása, majd a bőrhiány felkarról vett félvastag bőr és a fül elől és mögül nyelezett, kettős bőrlebeny kombinációjával kialakított pótlása.
Következtetés: Az esethez egyedileg adaptált sebészi módszer a szerzők saját és az irodalomban is leírt műtéti technikák ötvözete. Az indikációt és az alkalmazható technikákat az atrézia etiológiája, a hegszövet radikális eltávolítása utáni hámhiány nagysága és lokalizációja határozza meg. A beteg együttműködése, a rendszeres kontroll és az utasítások betartása elengedhetetlen fontosságú. A beavatkozást követően nagy torzító hegekkel a műtéti területen nem kell számolni. Bőrgyógyászati lokális, hámosító kezelés a gyógyulást gyorsíthatja.

Combined flap solution of auditory canal atresia caused by caustic damage – a case study
Summary
Introduction: There are numerous surgical techniques available to treat auditory canal atresia caused by a variety of congenital or acquired etiologies (chronic irritation, iatrogenic damage). We propose a comprehensive surgical reconstruction method for caustic damage and resulting atresia affecting the concha, and complete auditory canal in our current case report.
Objective: Our goal was to design and implement a unique surgical method that is suitable for the aesthetic reconstruction of an injury of entrance of the auditory canal, the tragus with a large area of epithelial loss, and for the resolution of scarred middle and outer auditory canal atresia and development of functional contouring of the affected areas.
Method: After removing the scar stricture, widening the bony wall of the ear canal with a drill, then replacing the skin defect with a combination of half-thick skin taken from the upper arm and a double skin flap taken from in front and behind the ear.
Conclusion: Our surgical method, uniquely adapted to the case, is a combination of surgical techniques already described in the literature. The indication and applicable techniques are determined by the etiology of the atresia and after the radical removal of the scar tissue the size and localization of the epithelial deficiency. The cooperation of the patient, regular control and compliance with the instructions are essential. After the procedure, there is no need to expect large disfiguring scars in the surgical area, dermatological local exfoliation treatment can speed up healing.
