Az arcideg anatómiai és funkcionális ismerete a fül-orr-gégészeti gyakorlatban kiemelten fontos.
A körülbelül 7-10 000 rostot számláló, kevert funkciójú ideg perifériás károsodásának számos oka ismert. Arcidegbénulást okozhatnak infekciók, daganatok, traumatológiai, illetve neurológiai kázusok, de arcidegbénulás jelentkezhet szisztémás betegségek, valamint veleszületett rendellenességek következményeként is. Leggyakoribb oka a sokáig idiopátiásnak vélt Bell-féle bénulás, amelynek etiológiáját illetően egyre elfogadottabb tény a herpes simplex vírus szerepe.
A perifériás arcidegbénulással jelentkező betegek célirányos kivizsgálása és megfelelő terápiája kiemelt fontosságú a prognózis szempontjából. Az arcideg funkciójának megítélésére szolgál a House–Brackmann-skála, amely meghatározza a további vizsgálatok és a terápia menetét.
Közleményünkben a faciális parézis Klinikánkon alkalmazott, a nemzetközi ajánlásokon alapuló kivizsgálási és kezelési szempontjait mutatjuk be három demonstratív, jól dokumentált eset közreadásával.

The up to date guideline of the diagnostics and therapy for facial nerve palsy
In otolaryngology, knowledge of the anatomy and function of the facial nerve is of paramount importance.
The facial nerve with approximately 7-10.000 fibres performs multiple functions. Peripheral lesion of the nerve can occur due to many local conditions like infection, tumour, trauma, neurological agents, but can also be the consequence of a systemic disease or congenital anomaly. The most frequent cause is Bell’s palsy. Until recently this condition was thought to be idiopathic but the role of herpes simplex virus has already been justified.
For a good prognosis, proper diagnostic and therapy for patients with newly developed facial palsy is essential. The House-Brackmann scale is a valuable tool to evaluate the nerve function and determines the subsequent examinations and treatment.
In our publication, we summarize the up to date guideline of the diagnostics and therapy for facial nerve palsy used at our Department. Our diagnostic and treatment methods are in accordance with the international standards. With the demonstration of three well documented cases we draw attention to the potentially successful treatment strategy.
